Can a family member complete it?

A family member can help organize questions, but the client and applicable authority still need to be addressed. Family relationship alone does not decide preferences, access, or update permission.

Does the preference record guarantee the same cleaner?

No. Continuity can be recorded as a preference. Staffing, substitutes, contact-first choices, and rescheduling options depend on the approved plan and availability.

Can this record describe mobility items?

Yes, when the instruction is cleaning-relevant, such as an item not to move, a cord location, or a path to keep clear. It should not contain diagnoses or ask the cleaner to provide mobility assistance.

A calmer first contact

Review every answer and keep a copy when email does not open.
